Register With The Practice?
If you wish to register with the practice and you live within our practice boundaries (see the practice boundaries map), please call into surgery to book an appointment collect any forms
relevant to your registration. You will be registered with the practice and have the right to choose a GP of your choice.
You will be asked to complete a new patient questionnaire (this just asks for some basic information about yourself and your medical history). Should you wish to have a health check please make an appointment with the
healthcare assistant (the health check consists of checking your blood pressure, height, weight, medical history, smoking history).
Children under five years of age can be registered for child health surveillance. In addition you will be asked to provide details of childhood immunisations of children under six years of age (this will ensure your
child is included in the computerised recall system).
It is recomended that all patients over 40 years have regular blood pressure checks. Please book an appointment with the healthcare assistant.
How Do I....
Change My Details?
If you change your address or telephone number, please inform the receptionist as soon as possible. This helps us to keep an accurate record and ensure any correspondence goes to the correct address.
