How Do I....
Obtain A Repeat Prescription?

Ways Of Ordering

WE DO NOT TAKE PRESCRIPTIONS OVER THE TELEPHONE.

If you require a repeat prescription you should either order your prescription by post or by hand. (You can also fax your request through to the surgery on 0161 226 5644.)

You can place your prescription request in the box provided in surgery or you can post it to us.

If you would like your prescription posted back to you please provide us with stamps or an SAE. Please remember to order your prescription in plenty of time in order to avoid delays with the postal service.

Know Your Medication

Make sure you know the name of your medication when ordering as this will prevent mistakes being made.

Please use the white part of your prescription for ordering. Just tick the items you require as this will save you having to write out a request form.

Time

  • Prescriptions will normally be ready within 24 working hours.
  • Prescriptions ordered before 3.00pm will be ready after 1.30pm the next working day.
  • Prescriptions ordered after 3.00pm will take two working days.

Change Of Medication

If your medication has changed eg through the hospital, please inform the staff or doctor of this change. Also, where possible, please provide the pink medication slip provided by the hospital.

The Prescription Form

Your prescription form comes in two parts:

  • The left-hand GREEN part is for you to give to the chemist.
  • The right-hand WHITE part is for you to keep.
  • The WHITE side of your prescription has a list of all medication currently authorised for repeat issue. When you require your next prescription simply tick the box next to the items you require and drop this off at the surgery reception.
  • Your new prescription will be ready after 1.30pm the next day (if received before 3.00pm).

Review

Everyone on repeat medication should have a review at least once a year; this will normally be done face to face with a doctor or nurse.

On the WHITE half of your prescription is you will see your personal review date. You will need to make an appointment with a doctor or nurse for a review of your medication before that date.

Remember:
Always retain the WHITE side of your prescription to order your next repeat. Handwritten requests can cause unnecessary delays and potentially serious errors.

Always read the information on the WHITE side of your prescription. This can include your review date and other messages to you from the surgery.

If you have any queries about your repeat prescription, please speak to one of the reception staff.

How Do I....
Obtain A Sickness Certificate?

Self-certificates are available from reception and can also be collected from your local DSS office; these can be used if you are absent from work for less than seven working days. If you are absent from work for seven days or more you will require a medical certificate issued by the doctor. Issuing private certificates for employers, school or college is a non-NHS service. There could be a charge for issuing these.
